Planning for Peace: Medical Power of Attorney Setup & Workshop

Do you have a Medical Power of Attorney? Every adult over the age of 18 should have one.

A Medical Power of Attorney (MDPOA) is a vital document that ensures your healthcare wishes are honored if you become unable to make medical decisions due to illness or accident. By appointing a trusted agent, you give someone the legal authority to make medical decisions on your behalf—helping prevent confusion, family conflict, and court-ordered guardianship.

What to Expect:

  • MDPOA forms will be provided
  • Forms can be completed, signed, witnessed, and notarized on-site
  • Guidance on choosing a primary and secondary medical decision-maker

Before attending, please consider who you would trust to serve in this role and confirm they are willing to carry out your wishes.

This workshop is led by Debra Kooser, MA, a trusted member of our church community and an Elder Care Clinician and Specialist, offering compassionate guidance for life planning.
