A Medical Power of Attorney (MDPOA) is a vital document that ensures your healthcare wishes are honored if you become unable to make medical decisions due to illness or accident. By appointing a trusted agent, you give someone the legal authority to make medical decisions on your behalf—helping prevent confusion, family conflict, and court-ordered guardianship.
What to Expect:
- MDPOA forms will be provided
- Forms can be completed, signed, witnessed, and notarized on-site
- Guidance on choosing a primary and secondary medical decision-maker
